  • Once you have signed your care plan and are under care at Align Well, you will gain access to the portal. You should receive an email notification providing your portal credentials and prompting you to set up a password before logging in.

  • We believe its important to provide you accessible content and knowledge to help support you on your wellness journey. Our app does just that! Take it on the go or deep dive into it more by accessing general wellness information and symptom based courses to achieve your goals and serve your family!

  • You will be able to access wellness courses, your evaluation results, at home care recommendations and symptom-tailored referrals to better support your journey.

  • Yes! You will have direct access to contact your FGP in the menu bar of your app!

  • Unfortunately, your app access is dependent on your care plan (AWC practice members) and employment/enrollment (Align & Co. client) status. Once your plans are terminated or abandoned, you will lose access within that calendar month.
